I would like to make a questionnaire

I want to make a questionnaire with an area under each question for employee responses.

I've already got the PDF, but don't know how to add the fillable space below.

Do you mean to say that you want to add question in form where user can add responses? You need to edit your form and add desired questions. Please check the screenshot below:
