Client is blocked from filling out form

    For some reason the urgent question I posted here disappeared.  

    Client is receiving this message when they try to access our Jotform:

    Looks like your IP address has been banned because of a possible phishing attempt. If you believe that there is a mistake, let us know using the form below.

    However, the form asks him for his Jotform username - he does not have a Jotform username because he is supplying the information (trying to fill out our Jotform) not collecting the information.  The form is ours not his.  

    Our client needs to be able to fill out this form. We have literally hundreds of clients who will be using this form and we cannot provide all of their IP addresses. 

    We need to understand how we can prevent this problem from happening by understanding the root cause.  We are not collecting any passport numbers nor any financial information etc.  so we are NOT violating your terms of service.

    I have checked your account and you're in good status.

    Can you please share the IP address of the form user so that we can check and add it to our white list?
