I noticed the silver and bronze level have different numbers for the amount of 'forms' you are allowed to have. Silver being 25.
What is considered a form?
Let's say I need forms to have someone send their name and address to my email.
Is Name and Address, with two seperate blocks considered two forms?
Or is that all under one form?

Form is a collection of all fields added into it.
In the example that you have quoted, you can have one form containing Name and Address fields. The Name and Address are two separate fields in the same form.

So for the Silver plan you can have 100 such forms and for the Bronze plan you can have 25 forms.
