Form not forwarding

    My form is not forwarding.

     

    It's the one labeled Personal Information.  This has happened in the past.  It is important that I receive this form as it comes in. 

    This form needs to be forwarded to newbie@manikinmodels.com

     

    I have tried to delete and set up a new one but it still doesn't come across. 

     

    Please correct this and let me know when it is done.

    Reason(s) : 5.1.0 - Unknown address error 550-'5.1.0 <0000013e18d2b35c-4f7f064d-a890-40= d2-b4e3-b6fee2130dc2-000000@amazonses.com> sender rejected. IB504'
