Hi, i cant remember if i asked this or not. I have a form. I want to put in unit number in the first box where it asks.  Then i can complete the comments and photo upload section.

Then if i have another 10 jobs to do in the same unit I'd like to either have a box to tick which would automatically create the same 3 questions again.  Unit, comments, photo upload.  But id like the unit number input in the first question populate to the second, set and then for as many sets of items required.

Then i will link to Monday.com and have them.all load as individual tasks.

Is t h at achievable and can you please help m e with the form amendment or creation.

The above is the form Monday.com created but everytime we input a task we keep having to repeat the unit number.  But it does allow multiple photos to be uploaded

    Thx

    Daniel

    Hi Daniel,

    Thank you for contacting us.

Then if i have another 10 jobs to do in the same unit I'd like to either have a box to tick which would automatically create the same 3 questions again.

To clarify, do you mean to be able to add jobs or fields using the same Unit Number? If so, I think the best workaround is to embed a form inside a form using the iFrame embed widget. So the main form contains the Unit Number then the 2nd form contains the job or task. Then create a button that will trigger to show the 2nd form to add a task.

But id like the unit number input in the first question populate to the second, set and then for as many sets of items required.

I think the best approach to meet your requirements is to use the Populate IO application of JotForm. The application can prepopulate the form via URL parameters.

    For your reference: https://prepopulate.jotform.io/

Kindly give me some time to create a demo form for you.

    Apologies for the delay.

    Here's the demo form: https://form.jotform.com/193471030226954

    1. I created two forms, the Maintenance Form 1 will pre-populate the Unit Number on Form 2.

    Form 1: https://form.jotform.com/193471030226954

    Form 2: https://form.jotform.com/193481592850971

Form 1 contains a new task to fill in the fields then form 2 contains the form that has existing Unit Number.

    For your reference: How-to-Automatically-Pass-Form-Data-to-Another-Form

    2. I also added the 2 Submit button for a NEW task and to ADD a task.

    3. Then I added the necessary conditions to meet your requirements:

    Hide-or-Show-Multiple-Fields-at-Once

    How-to-Change-Thank-You-Message-Using-Conditional-Logic - please follow item 4 onwards.

You can clone the 2 forms for your reference: How-to-Clone-an-Existing-Form-from-a-URL
