Where are my forms?

I created an application for my animal rescue that two people already filled out.  Now I go to send the application to another person and it says I have no forms.  I was going to sign up to upgrade but not now if all my forms are lost. Please help.

Are you referring to the form https://www.jotform.us/form/93388326598172? I see that the form is associated with a different account Lovedbeyondbelief while you are now logged into dawn_m_peters which is registered using a different account. 

Please logout and login again to Lovedbeyondbelief so that you should be able to manage your form.
