Create forms

Hello,

  I am needing a form that they give their credit card information but I don't have a product.   I also need things like birthdate, anniversary, Passport number and expiration date.   Is this something you can make?

As to your current inquiry, those form requirements you mentioned are available in the form.

You can use them on the form as long as the form complies with the JotForm Terms of Use.

For the credit card information requirement, you will need to use Payment integration for it.
