how can i add custom source code with jot form?

    Below is my form but it is not working in jotform (boxes options are not working)

    Responsive Form

    Resize the browser window to see the effect. When the screen is less than 600px wide, make the two columns stack on top of each other instead of next to each other.

    Sorry for the inconvenience caused to you but I am unable to find the form you are referring to.

    Upon checking your account, I found that there is no form named "Responsive Form". 

    Could you please help us to understand the issue clearly by giving us more details. If possible, could you please attach the screenshot.
