After publishing my site, the form disappears in smaller windows

The form works great but as browser sizes shrink, it disappears. You can see it happening here:

It happens at about 560 wide.

So, it looks and works fine in desktop and most tablets but is not showing up on phones.

I was able to replicate the mentioned issue on my end. Can you please try embedding the form using the iFrame method?

Thanks, jherwin. Using iFrame worked although it is still glitchy and wouldn't center on the page. I used full browser width and it works OK that way, although that wasn't what I wanted to do.
