How can I create a FORM in microsoft word instead of a PDF?

    I want to past the results of a Jot Form into an electronic medical record (EMR).  The EMR does not have the ability to receive a PDF, only Microsoft word document.  Is there any way I can create my forms as a Microsoft document?

    Unfortunately, we do not have an option to create or transfer the Microsoft Word files as forms.

    It may be possible to copy and paste the form content to the Microsoft Word document. However, you'll not be able to get the same copy as the form view in the document and cannot receive any submissions.

    Thank you for the response.  I looked at the 2 options you proposed.  If I simply want to take the JOT FORM PDF and convert it into a word document, which is the better option?
