Trying to create a form for sales staff to upload "proofs", enter job number and email to client for approval

    This may be rather basic but I am stumped.  I am trying to create a form for sales staff to upload "proofs", enter job number and email to client for approval. The proofs are in PDF format, the job#'s will be different for each proof / client. The sales people are not the type to open a jot form and do a bunch of designing - I need a process that is as simple as possible.

    You may add the fields you need.
