Why does my form not display properly?

    After emailing or embedding the form, it displays additional entries after the Statement: My child's immunization shots are complete and up to date Yes or No

    At this point the form renters Nickname and several other entries.


    How can I correct this problem? I need help ASAP since I need to post this form.

    Hi Billy,

    Do you mean that when you received the submission through email, there are few fields that are not present on the form? If so, I would suggest you to re-create the Form Notification.

    First Step: Delete the existing one - How to Delete Notifications/Autoresponder

    Second Step: Create another one - Setting Up Email Notifications


    However, if you mean that the form do not display properly to your webpage after embedding. I suggest you to re-embed the form using our iframe method: How to Get Form iFrame Codes. After that, clear your browsers cache so you can see the changes.
