Sharing with company

Hello!

I was working on a form for my place of work.  We had started a form under our work account and it was shared with me to edit.  Once I was done with that form I began working on a new form.  The system kept interrupting me asking for me to login.  I clicked the Login with Facebook option and now the second form is under my own account.  I need to transfer this form to my work account. How can I do so?

May we know the work account email id/username? Also, to which account of yours, it as shared? May we know your email id to which it was shared with you.

Is the email id the same as your current account email id holisticrebelution@gmail.com?
