Why my account is suspended?

Hello, I only requested that my shared template be removed. However, my
account is now suspended. Can you share with me why and how to correct this?

Your account was suspended as the forms in your account collecting social security numbers and also requesting ID uploads from the users which is against our Terms of Use.

If your purpose is not to collect SSN from the general public, but from your own customers you comply with recommendations below, then we can make the exception:

1. Add the Terms & Conditions Widget for your users to read and agree.

2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

This will make you the sole responsible for collection and handling. Also, as it a free account, you cannot collect ID uploads from the users.

We can only re-activate the account if you agree to add the required widgets and delete the ID upload field.

Or we can just delete the fields and re-activate your account.

Yes, only from my clients so I will add the wizards. Are they located where
I add everything else? That form doesn't have an ID upload that I'm aware
of but I will confirm and fix it.

There were several fields that violate our Terms of Use. We need to delete them before we reactivate the account.
