Let end user send a form to a specific email recipient upon completion.

This is my third time requesting the same thing. Can someone please give me a call 561-267-2350 for some help on creating such form?

If you want to let the user select the email address where a submission notification should be sent, you would need to:

1) List the email addresses in a Dropdown field.

2) Then create a new Autoresponder, and assign the Dropdown field as the recipient.

If you would prefer to list the Names instead of the Email Addresses, then create conditions to specify where the Autoresponder or Notification should be sent based on the person that was selected, please check this guide: https://www.jotform.com/help/167-How-to-Send-Email-Based-on-User-s-Answer
