I don't get notification email from user.
and when they submit form Thank you page display as blank page , only show url as http://submit.jotform.me/server.php
and user also don't get email confirmation (already check spam folder) .
BTW , I try testing both notification email and auto responder , it both come to my mailbox without problem.
What should I do? , help please...

On the Email Notification, try using email@example.com as the Form Sender Email:
On Auto Responder, change firstname.lastname@example.org to your own email address;
With regards to the thank you page, I can see the error. It's because of the captcha field on the second page after you hit submit button. I've look for the reason and found out that the Free Text HTML field under the Form Collapse "สำหรับผู้ปกครอง ลงนามอนุญาต" is causing this to happen.
To fixed that, simply copy the text from the field and paste it as "Plain Text";
Let us know for the results.

I follow your suggestion for both issues but the problem still exist.
For Thank you page , I even try delete that html field but still not work.
So I decide to create new form from some basic template
and everything is working like it should be now....
