    Why is template I customized not showing the new fields I created when the form is emailed to me?

    I created this form https://secure.jotform.us/form/31184193751151 from a template. The form shows up fine and allows me to fill in the fields, but what it emails me does not reflect the fields I created (see screenshot), but the original template's fields.

    You must have cloned the form and then implemented the changes in your form. You should delete the existing notification and add a new one.

    Adding a new notification will always include all the fields of your form in notification. Let us know if this solves your issue.
