I have whitelisted the IP address. One account under the IP and registered with the same email domain has created a form that collects banking information. We do not allow such as it goes against our Terms of Use (https://www.jotform.com/help/8-Terms-of-Use).
The username of the account is KarenYoskowitz. I've reactivated this account, so the user can edit the form and remove the said fields. Please advise your colleague to do this as soon as possible.

Please re-whitelist us and let us know how we can ensure this doesn’t continue to happen. Is there a phone number at which we could talk this through over the telephone? Thanks.

We currently do not offer phone support. I have whitelisted this IP address - 38.100.141.226.
