Hello,
I was looking at our form accessibility after the webinar and noticed that some of the question boxes are not displaying correctly. I'm not sure how to adjust that as the only parts that I can control are the questions asked and the colors. I attempted to adjust field size but that did not make the change. I would love an assist on this. If you look at the question "What initially attracted you to work at..." you'll see the display issue to which I'm referring because there is a line through the first two options. We apologize for the inconvenience caused. I was able to see the issue as well when I checked on your form.
Please allow me some more time to investigate and identify a fix. I will update you here shortly.

I found that it has to do with the corrupt Solid Theme template used by the form.
I refreshed the theme template and it appears to have fixed the issue.
Please check/test the form on your browser also.
Do let us know if issue still persist.
