Login Suspended

    Seems my account has been suspended. I'm assuming because I have a form titled "credit authorization". I did not add fields or send this to anyone but need my other form for clients for my business. Can you please reinstate quickly?

    This has been suspended again and this time I used your template. I need my booking info form for clients, as I sent it to 6-7 and now it shows disabled which makes me look completely unprofessional. Can you just remove the credit one and restore ASAP? I need to be able to work!

    I have deleted the offending form from your account.

    Then I reactivated your account. So, you can now login again and use your other form.
