Trying to copy a form.

Hello im trying to copy this form exactly https://form.jotform.co/90057191015853 but im struggling to find the same text boxes for example in this link  for full name you can have two rectangle boxes side by side to separate first name from surname.

DOB has a field that allows a calender pull down

There are also certain questions you can add asterick to make them compulsory to answer

Id like to add a custom picture in the same way this has too and large solid colour description box at the top

Please advise if the free version allows for all of this

any assistance would be greatly appreciated.

Also in my effort to create this one survey I clicked around and somehow managed to set up 5 surveys and no its no allowing me to delete or make new ones for the future, but I was trying to find templates that could start me off rather than making it from scratch

How about cloning the form? It should copy the exact text or the exact form.

You will find that you can specify what format you want a file to be even.
