    Can you send us the URL of the page that has your form? The form code might be missing some parts of it. The bullet point problem will happen when the CSS is missing.

    I only get the bullets when I drop the source code into a Template. I had to edit it in Dream weaver because I needed to add a input box in my paypal area of the form. when I view it in my browser its fine no bullets but the second I drop it into the template and view it bullets... I have done this before withe the jotform and this template and have not gotten the bullets... now I get them.

    I inspected your site's source code and found the css bit that places bullets next to your headings.

    div#mainContent ul {
        font-size: 0.8em;
        line-height: 1.5em;
        list-style-position: outside;
        list-style-type: disc;
        padding: 0 40px 0 45px;

    If you have access to the file text.css, find and remove the line in bold "list-style-type: disc;". That should remove the bullets in your form.

    You may also want to inject this css in your form:

    .form-header-group {
        clear: none;

    to remove the extra large space above your form. Refer to this page - Inject Custom CSS.

    Hope this helps. Feel free to post again if you have other questions.

    We use Jot Forms all the time in the websites we develop and love them.  Today is the first time I have run across the same problem as noted above from salonbeautec.

    No where on the CSS form or .html page is there a line that says list-style-type: disc.  However, I did add your fix suggestion to my CSS page - no fix and then to my .html page - still no correction.

    I still have bullet points beside each new category of the form.

    <iframe allowtransparency="true" src="//form.jotform.com/form/11034822808" frameborder="0" style="border-style: none; border-color: inherit; border-width: medium; width:75%; height:866px; " scrolling="no">

    I just uploaded the site I am developing to send you the url. As this site in the early development stages the CSS form is pretty short.  I could copy it and send to you if you like.

    Can you please check it out and advise how I can correct this?

    Hi Lenora,

    Thanks for bringing this to our attention. I tried to open the link you provided using four different browsers but can't seem to see the bullet points on each instance. May we ask which browser you're using?

    In addition, could you please do a minor change in your Iframe embed code and change the URL to the following:

    <iframe allowtransparency="true" src="//www.jotform.com/form/11034822808" frameborder="0" style="border-style: none; border-color: inherit; border-width: medium; width:75%; height:866px; " scrolling="no">

    Please let us know if you need further assistance. Thank you for using Jotform!
