Form shows blank.

    We are trying to embed the form onto Wix and end up with a blank box. Please help

    Apologies for the inconveniences caused.

    I've been testing the form and was able to replicate the issue on the direct form URL too, I will be performing some tests and will get back to you as soon as possible.
