PERSONALISED FORM

we are a video production company and we want to create a form with our T&C.s on to send to our clients before we attend their site and start work.

this is the form we have created

my question is that I want to fill the top section in myself before we send it to them and just have them click the bottom questions and sign.   how do i personalise this?  do i have to create a new form each time with the text changed at the top by me or can i start filling it out and send it half completed to the client?

You can prepopulate the form with the prepopulate app or Spreadsheet to form widget.
