I noticed that your form doesn't have a SUBMIT BUTTON. Your user won't be able to submit it if there is no submit button on the form:
Once the user submits the form, by default, you should be receiving an email notification.
You can also view the form's submission via the Submission Page. Please see this guide on How-to-View-Form-Submissions.

Thanks for your response. I am a total newbie at this. I love the form and how helpful it was to set it up. Just may have missed that step.

Sorry for the confusion.
First, if you're new to JotForm, I suggest checking this guide/video: https://www.jotform.com/help/2-How-to-Create-Your-First-Web-Form
It has all the information needed to run your form smoothly.
After that, make sure you add a notification email to your form. In this way, you will receive an email whenever someone submits to your form. Here's a guide: https://www.jotform.com/help/25-Setting-up-Email-Notifications
Lastly, you can view all submissions to your form on the Submission page. Here's how to open the Submission page: https://www.jotform.com/help/269-How-to-View-Form-Submissions
