I build a form ( http://form.jotformpro.com/form/22954655695974 ) to collect the response from my supplier
every day I will send mail to supplier with below link

But recently, I found there are some records which has lost some data e.g.wingsstaff, supplier, ... but these fields are locked, please let me know to check.

That could probably due to the & symbol which was converted to it's html symbol &. I can see that from you code, that makes the prepopulating of fields not to work. Please make sure you verified them before sharing the form URL.
Another possible reason is, the user probably changed something on the URL which causes the prepopulation not to work. Unfortunately, there's no way around to prevent this since the user has the control to changed it on on the address bar or on the source.
