    Olá.

    Atualmente utilizo a conta para fazer demonstração para clientes referente à integração entre os formulários criados com Jotform e a DocuSign.

    Podem verificar que não tem um grande volume e nem é utilizado com grande frequência, portanto não se trata de phishing.

    Gostaria de solicitar a reativação da minha conta e, para as próximas criações posso incluir a informação de demonstração, para que saibam que é apenas utilizado para apresentação.

    Unfortunately, your account was suspended because of your form (200162527816653) violated our Terms of Use. Upon review of your account, it was found that the Form has an ID field. So your account is suspended due to this.

    We will have to remove it for compliance. If this was a government-issued ID it needs to be protected by two mandatory widgets:

    1.) Add the Terms & Conditions Widget(or the Short Scrollable Terms Widget) for your users to read and agree.

    2.) Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    The widgets will apply for ID number collection. This will make you the sole responsible for collection and handling.

    I deleted the offending fields from your form and reactivated your account. Kindly login your account and make the necessary changes.
