When I receive my form submissions via email it is cutting off the birthdate.
How do I fix that? Also I would like a pdf of the form emailed too. Is that
possible? Please let me know.

You will need to re-create the form email template to be able to refresh all the necessary fields into the email template. The email templae is probably not update with edits/updates done on the form fields.
You can do this using the Add New Email button
Try this and test submit the form again to recieve the notification email. Check if this fixes the cut-off birthdate field.
You can have a link to the form's submission data in PDF format, to the email template. Use the PDF Link field available among the Form's Fields on the righ panel when in the compose email wizard screen.(see visual guide below)
