    Asking for login credentials is not allowed, we will need to delete those fields, also kindly provide us the following information in order to reactivate your account:

    1. The URL where your form is used.

    2. Your website or proof of affiliation to a legitimate institution.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    Please send these proofs to reviewers@jotform.com with Thread #2121449 as your subject. We will let you know about the status of your account after a thorough review of your proofs.

    I will not go through the trouble of sending you all anything. Please just
    delete this account and remove email from system. I no longer choose to use
    your services. I will pay to use adobe and not be patronized about the
    information required on my form.

    We do understand. However, please be aware that online form builders, including Jotform, are being used for identity theft in the past. We go to great lengths to prevent this so we do not allow Login Credentials such as Username or Passwords to be shared using the form.

    We can lift the suspension but we need to delete the said fields and ensure that you agree with our TERMS.
