Slides Duplicating, Out of Order

I'm dealing with a frustrating issue.

It appears certain slides are either duplicating or out of order on my form.  

Basically, the order of my slides in the BUILD section are NOT what end up happening on the published form.

Proper order:

1.   Select What was reconnected: User selects Garbage Disposal and Faucet
1b. New or Used Faucet
1c. New or Used Faucet
1d. Enter Notes

2.  Next Reconnect information....

Instead I have:
1.   Select What was reconnected: User selects Garbage Disposal and Faucet
1b. New or Used Faucet
1c. New or Used Faucet
1d. Enter Notes

2.  New or Used Faucet
2b. New or Used Garbage Disposal.
And it just keeps repeating.

I have 5 sets of the slides that say: "new or used garbage disposal" & "new or used faucet."  They are all supposed to remain hidden unless selected in the previous questions.

I am pouring through my Conditions, they are correct.  But, for some reason the form is either duplicating slides, OR its not hiding the slides once the form is live.

Please help.  This is very frustrating.  The form was tested before being deployed.  It's first use in the field bungled a $5k job for us because it suddenly did not function properly.

I was able to reproduce the issue you have described when I tested your form. Can you try if emptying conditionally hidden fields as they are hidden would solve the issue you are having in your form, please?
