Account Suspended

    My account has been suspended due to possible phishing. I know why this happened; it's because I'm trying to create a form that supplies a username and password. So the system flagged it as phishing. If someone could unlock the account I'll be sure to change how those fields are presented. If you have any questions please don't hesitate to reach out.

    As checked, your account has been auto-suspended by our system as this form: Surveillance Account Summary contains plain text fields that collect Password/Login credentials. It is considered as phishing and violates our Terms of Use. We understand that this may not be your intention but the term "Password" is not allowed to be used on forms.

    You agree that JotForm, Inc. may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

    We can reactivate the account but we need to delete the infringing fields from your form. You must also agree with our Terms of Use.

    We have deleted the guilty fields and activated your account. Please refrain from doing it in any of the forms again.
