I was in the process of creating a new form to test a conference registration and somehow the form caused my account to be locked.

Upon review of your account, I found that it was automatically suspended by our Phishing Detector Tool (auto suspended)
Since forms are created everyday for phishing activities such as: stealing credit card information, social security numbers, login credentials, child pornography, etc; our Phishing Detector Tool automatically suspends accounts that may contain words that can seem suspicious or related to those activities.
I have inspected your forms manually and found no guilty fields, so I have activated your account, we apologize for the inconveniences this may have caused you.
You may continue developing your forms according to our Terms of Use.
Regards.
