Not sure why my account was suddenly suspended.

I created a form for my students to register for a piano competition and suddenly my whole jotform account is suspended.  It's realy important that the students sign up for the event soon. PLease fix.

The account was auto suspended by the Phishing detector due to the log in form https://www.jotform.com/form/200186598915162

We can only reactivate the account if you comply to the Terms of Use and agree to delete the form.

Ok - so how do I delete the form if I”m not allowed to access my account?
I know the terms and conditions, which is I why I decided not to use the auto form builder to create the first version of my form. I will need for you to delete it from your end then so that I can proceed.

I found the form with the said password field in your account's Trash folder — [Imported Form]. I have deleted the password field and reactivated your account.

You should be able to login to your account.
