widgets in my quiz

Hi,

I keep opening my questionnaire and random widgets get added on randomly. "BOOK FREE CONSULTATION" when you preview the form the  First screen where it says sport, birth year, birth month... I did not put this on and cannot get rid of it. 

Can you please remove asap or why is this happening?? 

I am not sure why is that widget shown on your form. It looks like a custom widget. Have you maybe created that widget? I checked your form and inside of the Form Builder, the Vimeo widget is shown instead of that widget. I am also unable to find any custom widget on your account. Have you maybe cloned that form? 

I cleared your form cache but the issue was not resolved. You can try to remove the Vimeo widget and re-add it to see if that will resolve the issue. 

I also clone your form and I was not able to replicate the same issue. Here is my form - https://form.jotform.com/200193815893965

You can also clone my form or your form to see if that will resolve the issue on a new cloned form.
