Form is not printing out correctly

Hi!  Our jotfom isn't printing out all of the information submitted by the patient when the attachment is downloaded.  Does this happen if the question doesn't have an answer?   This is happening with a few of the questions.  

You can uncheck the option Auto Hide Empty Fields in the form Submission option.

This will then show all the data on the submission even for those fields that do not have answers.
