Suspended Account

    My account was suspended "due to possible phishing." I checked over everything I could find before creating the form or account since I am needing to use the form as an order form. I made sure the form was encrypted as well. What do I need to do so that I can get my account and form active again? And if I make another for for another property of mine, would this happen again?

    I can see that your account had been suspended

    You have form on the account that is suspicious of phishing activity as it collects SSN and License Numbers. This violates our Terms of Use.

    If your purpose is not to collect these data from the general public, but from your own customers, then we can make an exemption provided that you comply with the following requirements:

    1. Add Terms of Use widget (for your users to read and agree). This will surely make your forms not to be considered as guilty of phishing activities

    2. Add a Signature widget where your users sign and agree to provide such sensitive information. (I see you already added this one.)

    3. If possible, change the email address on your profile to one that shows your business domain rather than the public email address you currently show.

    4. We need to delete also the passcode field on the form.

    We can reactivate the account but we need to remove these fields and ensure that you agree with our Terms.

    I will add the correct widgets and please remove the passcode field so I can get back up and running.

    Once the account is active again, will that also unblock my IP address?
