Imported PDF fields not recorded

Some of my fields in the fillable pdf that I uploaded were not registered on upload. Would I need to reload the form or is there a way to duplicate the fields that are on the pdf?

You can add a Short Entry text field and then map it to those fields which are missing the connection. Please check this GIF:

Im sorry I wasn’t clear. Some of my checkbox tillable fields didn’t
transfer over so there is no actual field on the pdf, see the Disability
    section at the bottom.
That being the case do I need to just upload a new pdf. I corrected the
fields in the pdf so that the names aren’t common thinking this would help.
    Your thoughts?

It seems that most of the radio button on the Form is not mapped to the Fillable PDF.

So you have to either add the radio option manually or you may also try importing the PDF manually to the form.

    Thanks

If you want to make the checkbox into single selection then please refer to this guide: How-to-Limit-the-Number-of-Selections-on-a-Checkbox-Field

Let us know if you are referring to something else.

Thank you, this should do the trick for me! This is actually how I have the
fields usually set up in the PDF, and use a script to control the
yes/no/other single check options so that I total control without using
    Radial buttons.
Unfortunately when importing it eliminated the same name fields so I had to
rename the fields for the JotForm to recognize the fields. I am hoping I
get the planned results because I see this being very useful for the
    organizations I consult for.
    Thank you,

Do let us know if you're still having issues. However, please note that the import PDF is still on it's Beta phase, as such, there might still be some limitations.
