Is it possible to setup both checkbox and a text field to select?

Wonder if it is possible to set up a form like attached screenshot. They choose which time suits them, and write time below?

You may try setting up an Input table field on the field with multi-type option.

The user is not required to Select the specific day since they can enter the text wherever required.
