I want to create a pdf page and fillable form

I want to create an application form with

Page 1

Name

address

phone number

(Which the client can fill in)

then second page i would like a pdf page as this will be a disclaimer and my terms and conditions and next page will be an invoice page which again i do not want them to touch so essentially a pdf page again. Is this something I can do, some fillable pages and some they can't touch?
