Why is my account suspended?

Whenever I try to login, I'm told my account is suspended. I've changed my password twice now. I'm not sure why it would be suspended. I can't get in to change anything.

Your account is suspended because your Pre-Paid Card Sales form is used to collect credit card info with pain text fields, which is against our Terms of Use

Card number, expiration date, security codes and card holder name are considered critical information and cannot be collected on plain text fields. The only services allowed to collect such data are those that are DSS/PCI certified.

If you want to collect payments, you need to use one of our payment integrations.

We can activate your account only if you agree with our Terms and allow us to delete the guilty fields.

form account for years. I dont recall getting an email about this. Hiw can
I get access to my account? You didn't give me any info on how to regain
    access.

Since forms are created everyday for phishing activities such as: stealing credit card information, social security numbers, login credentials, child pornography, banking information, etc; our Phishing Detector Tool automatically suspends forms that may contain words related to such activities.

I have now deleted the offending fields from your form and activated your account. Please refrain from creating such forms or adding any guilty fields to the forms to avoid any further suspensions on your account.
