Hello, I'm currently using the Spreadsheet to form widget. I keep receiving this error message, "There are errors on the form. Please fix them before continuing". Please assist.

Upon checking this form https://www.jotform.com/form/200214564846150, it shows that you have uploaded a .doc file and not a xls format.
The widget requires an .xls, .xlsx, or even a .csv file and it won't accept any other format. Please upload the required file format and test again.

Thank you for your response. Unfortunately, I’m still having the same issue. I’ve changed the formatting of the spreadsheet, so it’s no longer a .doc form. Once saved, my form is continuing to not populate the necessary field that is connected to the spreadsheet to form document. Below I’ve copied the spreadsheet to form widget and my conditions for that widget.

I would suggest you use the .xlsx extension for your spreadsheet.
