PDF Form Coverter: not all fields in the PDF form are being converted and mapped to form fields

    Can I get a form created? I am having trouble getting the form to look like the PDF I uploaded.

    Please note that when you import PDF form, the look & feel of imported form will not be exactly same. 

    I did check your PDF form and it has fields on the same line. Do you want to build form something similar? Please check the screenshot below:

    I have built the form, but my pdf is not importing all fields. How do I add fields on converted pdf in order to connect field to a form field.

    Please note that you can manually add question in your web form and then connect it with the PDF fields. Please check the screenshot below:

    Not all form fields in the pdf did not import. How do I add a field manually to the pdf to connect not in the actual web form

    If you are looking to add more fields in the PDF form that is connected with your online form I'm afraid that would not be possible with the JotForm builder page.

    What you can do is to create another PDF form, add new fields, then upload it to JotForm to connect with your online form.

    I have added new fields, even extras but they do not register to jotform no matter what I do. When I move fields around from extras I added the fields wont let me connect to it.

    Yes I would like my form to look the same as pdf if possible, but I have created a form already as well. Some links will not import into jotform pdf converter no matter what I do.

    Please kindly share the original PDF Document with us. For example, upload it to your Google Drive or Dropbox and share a link with us.

    Please also let us know what PDF fields are not being properly marked as fillable in JotForm editor.

    Patient's Birthdate first section - I can see this.

    None of the yes or no or any multiple choice questions - it is mapping but not all.

    Patient's Printed Name under patient treatment contract  - only the Patient's Printed Name is fillable on the original PDF. Please correct this part.

    Authorization account on file relation to patient and email - only email is not fillable.

    Authorization for bank (ach) relation to patient - it's fillable to my end.

    notice of privacy practices Birthdate - it is also fillable to my end.


    I can say it is not consistent, probably because of its size. 

    I have raised this thread to our backend team for further assistance. Once we have an update, you will be notified through this thread. 

    Please do not delete the PDF file on your dropbox folder.

    The bug report ticket is still open and marked as important.

    Our developers have recently added an option to link custom form fields to the PDF.

    So there is a chance that some elements can be manually fixed with this new 'Add to PDF' option. Can you please share your PDF file with us again?

    Unfortunately, the converted PDFs seem to be still having issues. We cannot provide an ETA on when this will be fully addressed, but we will let you know if we have any updates.
