Form over quota pop-up

    Hello,

    I am trying to complete UH rotator mandatory online training on Moodle, however when I click the section and read the module then attempt to click the link for attestation, a pop-up comes up stating 

    Form over quota!

    This form has exceeded its allocated quota.

    I have attached a screen shot. The Moodle IT directed me to here. How can I get assistance with this?
