Account suspended

My account for info@gwctr.com.tr seems like suspended but do not know why? Can you help me?

i embedded a jot form to my website GWCTR.com.tr and use this form getting applications to our Goldwing motorcycle Treffen 2020. 

Upon review of your account, it was found form id: https://www.jotform.com/form/200211990647957 was uploading Driving Licence. So your account is suspended due to this.

We will have to remove it for compliance. If this was a government-issued ID it needs to be protected by two mandatory widgets:

1. Add the Terms & Conditions Widget(or the Short Scrollable Terms Widget) for your users to read and agree.

2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

The widgets will apply for ID number collection or for ID upload collection. This will make you the sole responsible for collection and handling.

Please make sure to add the widgets before the button in order for the system to recognize a valid collection.
