When I send a form and receive a reply, not all the information is coming up

Hi,

I have been sending a form and everytime i send it and receive a response half my form isnt there but everytime i go on build the form everything will be there it is juts when it is responded too. Have I changed a setting or something. Dont know if you can see my form but it is under booking form, disclaimer, paypal.
