Is there any chance the a form could have stopped working over the weekend from April 9 to April 11?
I received a form on April 1st in the morning, and I checked to see if it was working on April 11 around 7:30pm. Between that time i should have received 10 to 12 submissions. Is there a chance I went over a limit on the free account? Can these submissions be recovered?

The form I am using is "Like us and win".

So far we haven't had any intermittent problems with form submissions in our users' accounts. Also, there is no possibility that you went over your limit, because if that's the case then your form would have been disabled by now and will only be activated next month when submission limits are refreshed.
It might be possible that traffic to your Facebook page was lower than the usual thus resulting to less conversions. Estimates are never 100% accurate.
