What if I want to add an existing form to the flash?

I think that's the same method. Can you please explain what do you mean by "add an existing form" ? Do you mean, you want to embed the form directly to the flash page without using the lightbox effect or option?
By the way, this guide uses Adobe Flash Software.

Well, I already have a form and I want to add it to flash. Let me explain more.. I'm new to flash but my mate taught me the basics, I'm making a website on flash, so I have downloaded a template.. I already have a form here and I want to add it to one of my pages or at lesat, I want to have some good organized form codes but whenever I ask, they give form codes which shows (tables in the right side, then left, then middle. It's just messy, and it is like only 4 questions. Email, contact, name, age. But I want to add more questions. If it is possible, I really need either to just add the existing form or just have a black form codes, organized, and can add up more questions.. I hope I explained well and I hope that I wasn't asking too much. I'll appreciate your help.
