How can I hide certain parts of the form and show/answer these parts in a response email to each submission?

    I am creating a Time Off Request Form for my department in which individuals can submit for vacation, meeting time, comp day, or personal day. I would like for the sections below the submit button to be hidden from the person requesting the time off. When the form is submitted, I would like for our department manager to be able to complete these previously hidden sections (date received, status or request, and comment section) possibly in the same email that the form was originally submitted and received in and then forward this email to the individual requesting time off. This way the individual receives an email stating whether their request has been approved, denied, or waitlisted. Can you please let me know the easiest way to achieve this or something close?

    I have added another textbox (Managers Code) and use collapse field to hide the manager's section. And then create conditions email for the autoresponder email

     

    In the "Managers code" use the "managersonly" for the department manager to see the hidden section when the manager is editing the submission. There is an edit link in the notification email for manager's.

    Thank you very much for your assistance. I have been following along but for some reason I am not able to set conditions. This is what shows up when I attempt to:

    Any ideas why this is happening? I would like to set up a good amount of conditions and this proves difficult when the above keeps happening.

    Hi Amanda, it looks like you tried to link an image for us to view but unfortunately we cannot seem to find or see it to figure out what is happening.     Hi,

    Can you please specify which browser are you using, for us to check further? I am not able to replicate on the said problem. Also, I can see you already have a few conditions made are you still experiencing the said problem?
