Urgent help required to produce visual reports

I need to create a visual report for both the forms I have created. It has only produced graphs for the 1st and 2nd questions. 

You may only get the values of the fields in the visual report and create a graph.

To clarify, do you want to compare both forms?

You may add more graphs or chart data per question.
