Suspended Account

    Good day


    I am failing to login into my account, says the account is suspended. Can someone assist in lifting the suspension.

    Hi Nowel - As checked, the account has been suspended as this form contains an email address and copyright trademark "Crossfit Tanhiwa". This is considered as phishing and is against our Terms of Use.

    You agree not to display or use, in any manner, any of the Trademarks or copyrighted material without the prior written consent of Jotform Inc. All other third party trademarks, service marks, logos and trade names appearing on any services which are part of the Jotform Service are the property of their respective owners. 

    In order to reactivate your account, we need to delete the form.

    Or kindly provide us the following information:

    1. The URL where your forms are used.

    2. Your website or proof of affiliation to a legitimate institution (CrossFit).

    3. Share with us a document that will prove that you're authorized to use the said trademark and email
